Please fill out the below form if you wish to drop your dancer from a class or withdraw from all classes.

**Please remember — any DROPS or WITHDRAWS must take place by the 25th of the prior month, e.g. if you wish to discontinue or drop a class for February, you must withdraw by end of day January 25th. It is your responsibility to drop classes before the deadline.
